Dear Applicant:

Thank you for considering the Wyandot Senior Village for your new residence.
In order to be put on our waiting list, we have certain requirements that you must meet. 

These requirements are as follows:

We must have proof of all income and medical expenses. If you do not have it you cannot be put on our waiting list.

Proof of income is verification of Social Security payments, interest from moneys, rental income, pensions, value of property owned etc.  If you have any questions about income and expense, please talk to our office.  All income must be gross.

Medical expenses include Medicare payments, medical insurance payments, prescriptions, over the counter medication that is an on-going item, doctor bills, hospital bills, mileage to go to doctors. A Doctor slip showing these are on going expenses and the prescriptions and over the counter items that the doctor recommended. Please remember that we must have proof of all medical expenses.  Again, if you are not sure what to include, contact us.

If you need help in filling out these forms, our office will help you.

Please make sure all blanks are filled out on the form in order to process you for the waiting list.  You will be sent a notice on whether you have been accepted or rejected.
